Office Policies & Procedures

Welcome and thank you for choosing Berman Physical Therapy for your physical therapy needs.

Florida currently allows direct access to outpatient physical therapy services for up to 21 consecutive days without a physician referral. If your treatment exceeds the 21 consecutive days, Florida Law and the State of Florida Physical Therapy Board requires patients to have a written referral from a licensed medical professional (MD, DO, Physician Assistant, Nurse Practitioner, Dentist, Podiatrist, or Chiropractor). It is your responsibility to obtain a referral as noted.

Cancellation Policy

Berman Physical Therapy requires a 24 hour cancellation as a courtesy to our therapists and clients. We reserve the right to charge a $75 fee for violation of this policy.

Payment

Berman Physical Therapy is a fee-for-service clinic. Your payment is due at the time services are rendered. We accept cash, check and most major credit cards.

Insurance Billing

We do not bill any insurance company. We'll be happy to provide a receipt with diagnosis and coinciding treatment codes if you choose to submit your bill to your insurance company. Berman Physical Therapy is not affiliated with health insurance providers and we have no control over the terms associated with your health insurance policy. Please contact your health insurance provider for details.

Privacy Policy

Berman Physical Therapy will maintain your privacy to the highest standards and may use or disclose your personal health information for the purposes of carrying out treatment, obtaining payment, evaluating the quality of services provided and any administrative operations related to treatment or payment.
